Why are airline pilots not held responsible for a crash, even if they missed a checklist item?

Along with that, what does it mean to “query” a flight?

2. Hello, Well...would be good to know who told you that. Since most of the aircraft accidents are actually attributed to "Human Error" (Blame on pilots). Maybe you are talking about only one specific accident where you think they should have blamed the pilots and they did not? I have no Idea what is to query a flight..
